Kheterpal et al identified factors predictive of postoperative renal failure in patients undergoing noncardiac surgery. These can help to identify a patient who may require more aggressive management. The authors are from the University of Michigan.
Patient selection: normal renal function preoperative, noncardiac surgery
Preoperative pedictors of acute kidney injury following surgery:
(1) age>= 59 years (HR 4.2)
(2) emergency surgery (HR 1.9)
(3) liver disease (HR 2.4)
(4) body mass index (BMI)>= 32 kg per square meters (HR 1.9)
(5) high-risk surgery (HR 2.9)
(6) peripheral vascular disease (HR 4.2)
(7) COPD on chronic bronchodilator therapy (HR 3.0)
Perioperative predictors:
(8) vasopressor therapy
(9) diuretic administration
