Pasero et al identified risk factors for candidemia in a patient in the intensive care unit (ICU) following cardiac surgery. These can help to identify a patient who may benefit from more aggressive management. The authors are from the University of Turin and San Giovanni Battista Hospital in Turin, Italy.
Patient selection: patient in the ICU following cardiac surgery
Risk Factors for candidemia:
(1) total parenteral nutrition (TPN; OR 9.6)
(2) severe sepsis (which may correlate with broad-spectrum antibiotic therapy; OR 4.2)
(3) SAPS II score (OR 1.16 per point, range of scores 0 to 160, OR 3.3 at score of 8)
(4) length of stay (LOS) in the ICU > 20 days (OR 6.4)
Risk factors for mortality:
(1) female gender
(2) acute myocardial infarction within the past 90 days (< 90 days)
(3) receiving dialysis
The mortality rate for a patient who develops candidemia was 31% at 30 days and 47% after 60 days.
