Randhawa et al reported risk factors for mechanical ventilation in children with Guillain-Barre Syndrome (GBS) in India. These can help to identify a patient who may require more aggressive management. The authors are multiple institutions in India and England.
Patient selection: 1 month to 12 years of age with Guillain-Barre Syndrome
Frequency of mechanical ventilation: 52%
Exam: MRC grading for muscle strength
All patients received intravenous immunoglobulin (IVIg) on admission.
Predictors on admission associated with mechanical ventilation:
(1) upper limb power <= 3 (OR 3.5)
(2) lower limb power <= 2 (OR 3.5)
(3) cranial nerve palsy (OR 3.2)
Predictors on admission associated with mechanical ventilation for > 21 days:
(1) upper limb power <= 2 (OR 4.2)
(2) lower limb power <= 1 (OR 4.5)
