Sharma et al reported predictors of 30-day readmission in a patient receiving CAR-T therapy. This can help to identify a patient who may benefit from more aggressive management. The authors are from Wayne State University in Detroit.
Patient selection: status post chimeric antigen receptor T (CAR-T) cell therapy
About a quarter of CAR-T therapy recipients are readmitted.
Reasons for readmission:
(1) cancer-related
(2) sepsis or infection
(3) neurologic events
(4) bone marrow depression (pancytopenia, neutropenia)
(5) hypotension
(6) hypoxia
Predictors of readmission:
(1) leukemia or non-Hodgkin's lymphoma (OR 2.4 for leukemia and 1.5 for non-Hodgkin's lymphoma)
(2) transfer to a facility (skilled nursing, intermediate care) at discharge (OR 1.5)
(3) chronic renal disease (OR 1.2)
(4) cerebrovascular disease (OR 1.8)
(5) noninvasive ventilation (OR 1.9)
(6) treatment at a non-teaching hospital (OR 0.52 to teaching hospital)
