Suragul et al identified factors predictive of a surgical site infection (SSI) following a pancreaticoduodenectomy. These can help to identify a patient who may require more aggressive management. The authors are from Mahidol University in Bangkok.
Patient selection: status post pancreaticoduodenectomy (Whipple procedure)
Predictors of an incisional surgical site infection
(1) preoperative biliary drainage (OR 3.0)
(2) postoperative pancreatic fistula (OR 2.8)
Predictors of an organ space surgical site infection:
(1) preoperative cholangitis (OR 10.1)
(2) postoperative pancreatic fistula (OR 6.5)
Obesity was also associated with organ space SSI on the multivariate analysis but had a p value of 0.06.
