Description

Wastfelt et al reported risk factors for pneumonia in patients following a stroke. These can help to identify a patient who may benefit from more aggressive management. The authors are from Orebro University in Sweden.


Patient selection: status post stroke

 

More important predictors of post-stroke pneumonia:

(1) age (OR 1.07 for each year increase)

(2) NIHSS stroke scale (OR 1.07 per point increase)

(3) dysphagia (OR 3.53)

(4) nasogastric tube (OR 5.29)

(5) mechanical ventilation (OR 4.65)

(6) COPD (OR 4.48)

Reference points for age and NIHSS are not given. In the implementation an age > 65 and NIHSS > 15 are used based on reference to the paper of Yuan et al.

 

Less important predictors:

(1) male sex (OR 1.42)

(2) diabetes mellitus (OR 1.15)

(3) smoking (OR 1.16)

(4) atrial fibrillation (OR 1.37)

 

The risk of pneumonia increases with the number of risk factors present.
