Description

Bayer et al reported the Prehospital Early Sepsis Detection (PRESEP) score for detection of a septic patient in a prehospital setting. This can alert EMT personnel to a patient who may require more aggressive management. The authors are from Jena University Hospital in Germany.

Patient selection: sick patient in the community, with emergency response team called

Parameters:

(1) body temperature in °C

(2) oxygen saturation in percent

(3) respiratory rate in breaths per minute

(4) heart rate in beats per minute

(5) systolic blood pressure in mm Hg

= SUM(points for all 5 parameters)

Interpretation:

• minimum score: 0

• maximum score: 11

• The higher the score the more likely that the patient is septic.

• A score >= 4 indicates a patient who may have sepsis.

Performance:

• For a cutoff >= 4 the sensitivity was 85% and specificity 86%.

• The area under the ROC curve was 0.93.
