Pacelli et al identified factors associated with mortality for a patient with an intra-abdominal infection. These can help identify a patient who may benefit from more aggressive management. The authors are from Catholic University in Rome.
Prognostic factors identified by multivariate analysis:
(1) APACHE II score
(2) hypoalbuminemia
(3) hypocholesterolemia
(4) Mannheim Peritonitis Index (MPI)
(5) preoperative organ impairment
Risk factors with an odds ratio of 2 to 3:
(1) presence of a preoperative organ impairment
(2) serum albumin <= 3.5 g/dL
(3) serum total cholesterol < 110 mg/dL
Risk factors associated with an odds ratio of 6 to 8:
(1) Mannheim peritonitis index > 26
(2) APACHE II score 11-20
Risk factor associated with an odds ratio of 25:
(1) APACHE II score >= 21
