Description

Sathitakorn et al reported a process for discontinuation of antibiotics in a patient with moderate to severe COVID-19 pneumonia. The authors are from Thammasat University (Thailand), University of North Carolina, Washington University, Mahidol University, and affiliated hospitals.

Patient selection: moderate to severe COVID-19 pneumonia in the ICU

Parameters:

(1) Clinical Pulmonary Infection Score (CPIS), based on the CPIS of Pugin et al for ventilator-associated pneumonia on admission

(2) serum procalcitonin in µg/L on admission

(3) CPIS on day 3

(4) serum procalcitonin in µg/L on day 3

drop in serum procalcitonin =

= ((initial procalcitonin) - (day 3 procalcitonin)) / (initial procalcitonin) * 100%

CPIS

Procalcitonin

Action

on admission: < 6

on admission: < 0.5 µg/L

antibiotics not started

on day 3: < 6

on day 3: < 0.5 µg/L

discontinue antibiotics

NA

drop over 3 days >= 80%

discontinue antibiotics

The actions were taken after consideration of clinical status and cultures.

NOTE: The CPIS was derived for ventilator-associated pneumonia. Presumably most of the patients were on ventilators, but they were unlikely to have ventilator-associated pneumonia based on the use of admission and day 3 values.
