Stocker et al used a protocol of serial laboratory measurements to exclude early-onset neonatal sepsis (EONS) with a high probability. This can allow for the discontinuation of antibiotic therapy within 36 hours of admission. The authors are from multiple institutions in Europe.
Patient selection: neonate with possible early-onset sepsis
Timing of testing:
(1) on admission
(2) 12 hours
(3) 24 hours
(4) 36 hours
Laboratory Test
Proven Sepsis
Probable Sepsis
serum CRP
>= 16 mg/L
>= 13 mg/L
serum procalcitonin (PCT)
>= 2.8 ng/L
>= 1.5 ng/L
If serial measurements are all negative, then EONS is unlikely.
