Han et al reported the ratio of the red cell distribution width (RDW) to serum calcium in patients with acute pancreatitis. This can help to identify a patient with severe disease. The authors are from Sichuan University in China.
Patient selection: acute pancreatitis with age >= 18 years
Pancreatitis
Clinical Features
mild
absence of organ failure and complications
moderate
organ failure <= 48 hours and/or complications (local or systemic)
severe
organ failure > 48 hours
Parameters:
(1) red cell distribution width (RDW) in percent
(2) serum calcium in mmol/L
ratio of RDW to serum calcium =
= (RDW) / (serum calcium)
Interpretation:
• A ratio > 7.04 identifies moderate or severe acute pancreatitis.
• A serum calcium < 1.995 had a similar Youden index (0.60) for moderate or severe acute pancreatitis.
Performance:
• The area under the ROC curve is 0.91.
