Description

Groothuis et al developed a score for evaluating a pediatric patient with a respiratory infection associated with respiratory syncytial virus (RSV). This score can help identify patients who require more aggressive management and it can be used to monitor patients following therapy. The authors are from multiple university hospitals in the United States participating in the RSV Immunge Globulin Study Group.

Parameters:

(1) oxygen saturation

(2) respiratory rate

(3) retractions, wheezing and crackles

Parameters

Points

oxygen saturation

baseline without signs of infection

0

baseline with signs of an upper respiratory tract infection

1

decrease < 5%

2

decreased 5-10%

3

decrease > 10%

4

assisted ventilation

5

respiratory rate

baseline with no signs of infection

0

baseline with signs of an upper respiratory tract infection

1

increase 1 to 14 per minute

2

increase 15 to 30 per minute

3

increase > 30 per minute

4

assisted ventilation

5

retractions, wheezing, crackles

none (no signs of infection)

mild

2

moderate

3

severe

4

The respiratory score is determined as:

(1) the mode of the 3 scores (if 2 or more are the same value, then this is the score)

(2) if there is no mode, then the score is the mean of the 3 scores

(3) a score of 0 or 5 would be the same for all 3 scores

Respiratory Score

1

2

mild lower respiratory tract infection

3

moderate lower respiratory tract infection

4

severe lower respiratory tract infection

5
