There has been some controversy with the use of radiocontrast material in patients with sickle cell anemia or other sickling hemoglobinopathies.
Cases ofvaso-occlusive crisis and/or hemolysis has been reported following the use of radiocontrast material in patients with sickling hemoglobinopathies.
Risk factors linked to the episodes:
(1) use of hyperosmolar contrast material
(2) dehydration
(3) high concentration of sickling hemoglobin
Preventive steps proposed:
(1) perform contrast studies only when clearly indicated
(2) use a low-osmolarity contrast agent
(3) make sure that the patient is well-hydrated
(4) reduce the concentration of sickling hemoglobin to less than 30%
