The risk of surgical site infection following gastroduodenal surgery is increased when certain factors are present. A patient at increased risk may benefit from prophylactic antibiotics, a surgical technique associated with reduced risk of infection, and close post-operative monitoring.
Pathophysiological factors associated with increased infection risk:
(1) reduced gastric acidity
(2) diminished gastrointestinal motility
Risk factors:
(1) gastrointestinal obstruction
(2) gastrointestinal hemorrhage
(3) gastric ulceration
(4) malignancy
(5) morbid obesity
(6) therapy with H2-blocker (ranitidine et al)
(7) therapy with a proton pump inhibitor (omeprazole et al)
where:
• I would imagine duodenal ulceration would also increase the risk.
