Description

Arsenio et al identified risk factors for surgical site infection (SSI) following orthotopic liver transplant. These can help to identify a patient who may benefit from more aggressive management. The authors are from Hospital Universitario Puerta de Hierro, Universidad Autonoma de Madrid, and RESISTRA-GESITRA in Spain.


 

Patient selection: orthotopic liver transplant

 

Rate of SSI: 8.8%

 

Most common types of surgical site infections: incision site infection and peritonitis

 

Risk factors for SSI:

(1) history of previous liver or kidney transplant

(2) performing a choledochojejunal or hepaticojejunal biliary reconstruction (versus a choledochocholedochostomy anastomosis)

(3) more than 4 units of blood transfused

 

where:

• A previous history of a solid organ transplant indicates that the person was immunosuppressed at the time of the transplant.

• 24% of infections occurred more than a month after the transplant.

• Choledochocholedochostomy involves anastomosing the transplant liver's bile duct to the recipient's bile duct.

• A history of previous liver transplant indicates a redo procedure.

 

Absence of antibiotic prophylaxis was not identified as a risk factor but is still indicated.
