Patient selection: patient admitted to the hospital for community-acquired pneumonia (CAP)
Outcome: 30-day readmission
Risk factors for a pneumonia-related readmission:
(1) treatment failure (hazard ratio 2.9)
(2) >= 1 instability factors on discharge (hazard ratio 2.8)
Treatment failure:
(1) hemodynamic instability
(2) respiratory failure
(3) need for mechanical ventilation
(4) progression of the pneumonia on imaging studies
(5) appearance of a new infectious foci
Factors considered unstable at discharge:
(1) temperature >= 37.2°C
(2) heart rate >= 120 beats per minute
(3) respiratory rate >= 24 breaths per minute
(4) systolic blood pressure <= 90 mm Hg
(5) receiving supplemental oxygen by face mask or nasal prongs
(6) unable to eat or resume long-term tube feedings
Risk factors for a readmission unrelated to pneumonia:
(1) age >= 65 years (hazard ratio 4.5)
(2) Charlson comorbidity index >= 2 (hazard ratio 1.9)
(3) in-hospital decompensation of comorbid condition(s) (hazard ratio 3.5)
