Caupenne et al identified risk factors for mortality in an older adult with Clostridium difficile disease. This can help to identify a patient who may require more aggressive management. The authors are from Universite de Poitiers, Centre Hospitalier Metropole Savoie, Universite Victor Segalen Bordeaux 2 and Centre Hospitalier Universitaire Grenoble Alpes in France.
Patient selection: adult in the hospital with Clostridium difficile disease, age >= 75 years
Outcome: 30-day mortality
Predictors of mortality:
(1) one or more organ failures
(2) Cumulative Illness Rating Scale (CIRS) > 14 (as a measure of severe comorbidity)
Organ failure criteria:
(1) renal: use of intravenous loop diuretics or rise in serum creatinine concentration >= 1.5 times baseline
(2) pulmonary: need for oxygen
Criteria for other organ failures were not given.
