Description

Cordonier et al identified risk factors for predicting mortality from invasive aspergillosis in a patient who has had a hematopoietic stem cell transplant. This can help identify a patient who may benefit from closer monitoring and more aggressive management. The authors are from multiple universities and hospitals in Paris, Strasbourg, Nantes and Villejuif in France.


 

Patient selection: hematopoietic stem cell transplant, either autologous or allogeneic

 

Risk factors for death from invasive aspergillosis:

(1) age from 12 to 35 years of age

(2) dissemination of the aspergillosis

(3) pleural effusion present

(4) absolute monocyte count < 120 per µL

(5) prolonged administration of corticosteroids in the 2 months prior to invasion

(6) dose of corticosteroids >= 2 mg/kg at time of diagnosis of aspergillosis

(7) uncontrolled graft-vs-host disease (GVHD)

 

Patients with invasive aspergillosis had a high mortality even if the aspergillosis was controlled (page 961).
