Description

Emuakhagbon et al identified patients undergoing liver tranplantation who are at increased risk for postoperative deep vein thrombosis. A patient at increased risk may benefit from more aggressive management. The authors are from the University of Louisville and University of California Los Angeles.


Patient selection: liver transplant recipient

 

Outcomes: deep vein thrombosis and pulmonary embolus

 

Risk factors for deep vein thrombosis:

(1) intraoperative transfusion with multiple units of fresh frozen plasma (FFP) and cryoprecipitate

(2) elevated postoperative INR on postoperative day 7

(3) complicated postoperative course (respiratory failure, renal failure, hemorrhage)

 

where:

• The elevated INR on postoperative day 7 is an indicator of hepatic dysfunction although interpretation is complicated if the patient is receiving thromboprophylaxis.
