Patient selection: CLL, multiple myeloma, non-Hodgkin's lymphoma, Hodgkin's disease
Intermediate risk factors:
(1) more aggressive or intensive chemotherapy, including multiple lines of therapy
(2) relapsed, refractory or advanced disease
Low risk:
(1) conventional chemotherapy
Other risk factors:
(1) immunosuppression
(2) severe and prolonged neutropenia
(3) history of previous invasive fungal infection
