Peponis et al identified risk factors for a surgical site infection after a laparoscopic cholecystectomy. These can help to identify a patient who may benefit from more aggressive management. The authors are from Massachusetts General Hospital.
Patient selection: laparoscopic cholecystectomy, with half having acute cholecystitis
Outcome: surgical site infection (superficial, deep, intra-abdominal abscess)
Risk factors for surgical site infection:
(1) bile spillage
(2) conversion to an open procedure
(3) ASA status 3 or 4 (no ASA 5 in cohort)
The authors emphasized the need to avoid bile spillage during the procedure.
