Description

Ridgeway et al identified risk factors associated with surgical site infection (SSI) following hip arthroplasty. These can help identify patients who may benefit from closer monitoring for infection following surgery. The authors are from the Health Protection Agency in London.

Parameters identified on multivariate analysis:

(1) age

(2) preceding trauma

(3) ASA classification

(4) duration of surgery in minutes

where:

• The score is based on Table VIII on page 848.

• The odds ratios in the table range from 1.0 to 1.87.

• In the table above, an odds ratio < 1.5 was assigned 0 points and an odds ratio >= 1.5 was assigned 1 point.

• The rate of SSI was 2.2% in 16,291 total hip replacements, 5% for 5,769 hemiarthroplasties, 3.7% for 2,550 revision total hip replacements and 7.6% for revision hemiarthroplasties.

total score =

= SUM(points for all 4 parameters)

Interpretation:

• minimum score: 0 (corresponds to cumulative odds ratio of 1.0)

• maximum score: 4 (corresponds to cumulative odds ratio of 7.6)

• The higher the score the greater the risk of an SSI.
