Description

Rua-Figueroa et al identified risk factors for bacteremia in a patient with systemic lupus erythematosus (SLE). These can help to identify a patient who may require more aggressive management. The authors are from multiple institutions in Spain.


Patient selection: systemic lupus erythematosus

 

89% of patients were female.

 

Mortality rate: 14%

Recurrent bacteremia: 27%

Nosocomial source: 35%

 

Most common isolates: E.coli, Staphylococcus aureus, Salmonella

 

Risk factors for bacteremia:

(1) therapy with corticosteroids (dose-dependent)

(2) immunosuppression (mycophenolate, cyclophosphamide)

(3) diabetes

(4) cancer

(5) hepatitis C

(6) renal transplant

(7) active nephritis

 

Additional risk factors:

(1) SLE flare

(2) higher SLICC/ACR damage index (active)

(3) elevated serum creatinine

 

Factors associated with reduced risk:

(1) antimalarials

(2) dialysis
