Yli-Kyyny et al identified risk factors for early readmission due to surgical complications after therapy of a proximal femoral (hip) fracture. These can help to identify a patient who may benefit from more aggressive management. The authors are from multiple institutions in Finland.
Patient selection: surgical repair of a proximal femoral fracture (divided into femoral neck, peritrochanteric and subtrochanteric)
Outcome: early readmission for surgical complications (within 3 months of surgery)
Key risk factors:
(1) heavy alcoholism
(2) Parkinson's disease
(3) pre-existing osteoarthritis
(4) rheumatoid (autoimmune) disorder
Additional factors:
(1) fracture of the femoral neck
(2) depression
(3) presence of a psychotic disorder
(4) operative delay >= 3 days
(5) history of a previous hip arthroplasty
The hazard ratios were <= 2.0 indicating no single risk factor was predictive.
