Parameters:
(1) age of the patient in years
(2) diabetes mellitus
(3) significant cardiac disease
(4) significant pulmonary disease
(5) other organ dysfunctions/failures
(6) history of malignancy other than squamous or basal cell carcinomas of the skin
Significant cardiac disease - one or more of the following:
(1) angina
(2) coronary artery bypass surgery
(3) congestive heart failure
(4) ventricular arrhythmia
Significant pulmonary disease - one or both of the following:
(1) FEV1 < 1 liter/second
(2) PaO2 < 60 torr on room air
