Description

Systemic sclerosis may affect internal organs in addition to the skin. Medsger et al developed severity scales to grade severity of end-organ damage in the lungs and other organs. The authors are from multiple universities in North America and Europe.


 

Parameters:

(1) DLCO as percent of predicted

(2) FVC as percent of predicted

(3) bibasilar rales

(4) radiographic evidence of fibrosis

(5) pulmonary hypertension

(6) oxygen required

 

Severity Grades: 0 (none) to 4 (endstage)

 

Criteria for Grade 0 (none) - all of the following:

(1) DLCO >= 80% of predicted

(2) FVC >= 80% of predicted

(3) absence of bibasilar rales

(4) no evidence of fibrosis on chest X-ray

(5) no evidence of pulmonary hypertension

 

Criteria for Grade 1 (mild) - all of the following:

(1) NOT Grade 0, 3 or 4

(2) DLCO >= 70% of predicted

(3) FVC >= 70% of predicted

 

Criteria for Grade 2 (moderate): NOT Grade 0, 1, 3 or 4

 

Criteria for Grade 3 (severe) - both of the following:

(1) one or more of the following (OR):

(1a) DLCO < 50%

(1b) FVC < 50%

(1c) moderate to severe pulmonary hypertension

(2) oxygen not required

 

Criteria for Grade 4 (endstage): oxygen required
