Description

Kennedy et al reported the Sickness Assessment (SA) model for evaluating an older adult with a surgical emergency. This can help to identify a patient who may require more aggressive management. The authors are from Bristol Royal Infirmary and Royal Devon and Exeter Hospital in the UK.


Patient selection: adult >= 65 years of age with surgical emergency

 

Parameters:

(1) systolic blood pressure on admission

(2) presence of severe chronic disease (comorbidity) or compromised immune state

(3) self-care status

where:

• Severe chronic disease includes cirrhosis, angina on minimal exertion, renal disease requiring dialysis, pulmonary disease that restricts household work.

= SUM(points for all of the parameters)

 

Interpretation:

• minimum score: 0

• maximum score: 3

• A score >= 1 is considered positive (high surgical risk).
