Description

Black et al developed the SOFSURF risk score to help to identify a woman with osteoporosis. The authors are from the Study of Osteoporotic Fractures (SOF) Research Group and SURF stands for Study Utilizing Risk Factors.

Patient selection: postmenopausal female

Parameters:

(1) age in years

(2) weight in kilograms

(3) cigarette smoking

(4) history of post-menopausal fracture (or fracture after age 50)

where:

• The original abstract did not score age < 65 years. The study was done in women between 67 and 72 years.

• An age < 65 years yields negative points.

total score =

= SUM(points for all 4 parameters)

Interpretation:

• minimum score: -4 or lower (depends on age)

• maximum age: 12 or more (depends on age)

• The higher the index the greater the risk of osteoporosis.

• Black et al use a cutoff > 3 (which was the median score in the study).

• Koh et al used a cutoff of 1.4.

Performance:

• The score has a good sensitivity but low specificity, which is suitable for a simple screening tool.

• In the original abstract with a cutoff of 3, 85% of women with scores > 3 had a T score < -2.5.

• In the study of Koh et al, a cutoff of 1.4 had a sensitivity of 90% and specificity of 46% in a population of Asian women, with an AUC of 0.76.
