The Supervision Rating Scale (SRS) can be used to evaluate a patient who has had a head injury. This can help measure the extent to which a person can function either in the community or with supervision after a significant head injury.
Supervision indicates that someone is responsible for being with the patient.
Parameters:
(1) patient's need for supervision
(2) person providing supervision
(3) environmental controls
(4) extent of full time supervision, if needed
