Baptista et al reported a nomogram for dosing vancomycin for an adult in the intensive care unit (ICU). Proper dosing achieves therapeutic concentrations faster. The authors are from the Universitario de Coimbra in Portugal.
Patient selection: adult in the ICU
Outcome: daily dose administered by continuous infusion
Goal: target drug concentration of 25 mg/L
Parameters:
(1) creatinine clearance based on 8-hour urine collection in mL per minute per 1.73 square meters, from 30 to 350 (augmented renal clearance is greater than 130).

= (0.012125 * (creatinine clearance)) + 1.30625
Based on the data in Table 3:
