Babesia should be considered as a diagnosis if certain findings are identified.
Reasons to consider babesiosis:
(1) unexplained fever in someone who lives in an endemic area or who has traveled to an endemic area in the past 2 months
(2) unexplained fever with a history of blood transfusion within the past 6 months
(3) unexplained febrile illness following a tick bite
(4) serious febrile illness in an immunocompromised or asplenic individual
(5) deterioration in a patient being treated for Lyme disease or anaplasmosis
(6) ring forms seen on a peripheral blood smear
